How to make a referral

Contact us — Call the provided referral number for the specific facility of interest or complete the online referral form below. Our team will respond accordingly.

Step 2

Clinical review — Our admissions clinician reviews the clinical picture with you and confirms program fit. We'll ask about diagnosis, presenting symptoms, current medications, and support needs.

Step 3

Insurance & authorization — We handle insurance verification and prior authorization on our end, and communicate directly with payers. We will give you a clear timeline.

Step 4
