Assessments & Referrals

Starting the Assessment and Referral Process

The first step of the healing journey is letting someone know you need help. Perimeter is here to make this step as easy as possible.

Our approach initiates with a comprehensive assessment conducted by our trained professionals. We aim to gather pertinent information about the patient and create a personalized treatment plan. When you reach out to our assessment and referral team, we will pose a series of questions to facilitate the scheduling of a face-to-face appointment. During this in-person assessment, a mental health professional will conduct a thorough evaluation, delving into the patient's history, current medication, prior treatment, and more. This detailed information assists us in offering a recommendation regarding the most suitable course of treatment for the patient's unique situation, whether it involves inpatient care, outpatient services, or another treatment option.

Referrals

The assessment process can be as easy as a phone call or walk in for an assessment. Our assessment and referral team is available 24/7 to respond to your needs.

We are Here to Help

When you reach out to us, we will walk you through the process, answer your questions and help you take that first step. It all begins with a free, confidential assessment by calling 417-799-7474. We are available 24 hours a day, 7 days a week to help determine the best course of treatment for you or someone you love.

    Family involvement

    Communication

Our therapists will get in touch with you by email and/or phone to gather important information that helps improve the patient's treatment. It's crucial for you to respond promptly so we can give the best care. You will receive their contact info, so feel free to reach out if you have any questions during the patient's stay.

Our therapists invest significant time in delivering comprehensive treatment to the patients. They'll respond to your questions or concerns at the earliest opportunity. Your cooperation is essential for the patient's well-being.

    Family Session

Active family involvement in treatment has proven to enhance a patient's response and contribute to longer-lasting effects. We understand the multitude of responsibilities family members handle, and we kindly request your commitment to scheduling meetings with our therapists, as this plays a crucial role in the treatment process.

    Visitation

At Perimeter Behavioral Hospital of Springfield, we understand that a patient's family is a major part of healing and overcoming mental health challenges. We know many family members and parents fear not being able to see their loved one during treatment.

We offer scheduled visitation several times throughout the week. Please note because of the COVID-19 pandemic, visitation may be held virtually.

For the safety of our patients, no personal belongings are allowed in the visitation area. Lockers will be provided for visitors to secure their items. Also, no outside food or drink is allowed in the visitation area. Visitation is restricted to family members 12 and older. Only two visitors are allowed at a time per patient. Each visitor must be on the approved list, provide an ID or driver’s license and have the patient’s ID number.

Phone time will be also provided for patients to have contact with their families and support system. Please refer to the program schedule or contact the facility for more details.

    Discharge Planning

After treatment in our hospital, ongoing care will likely be required including a psychiatrist and therapist. It's a requirement to have follow-up appointments scheduled prior to the patient’s discharge. We ask that if you have preferred providers, please let us know who they are. We also request that you include the patient's primary care physician information. After treatment is complete, our staff will fax pertinent information such as psychiatric evaluation, medication information and the discharge plan to each of the next care providers. This ensures continuity of care and allows them all to be on the same page. We want the patient to have a smooth transition and to set them up for continued success.

    Standard Charges Price Guide

It is important to note, a hospital “charge” is not the same as “expected payment.” A charge is the amount billed for a service. In the vast majority of cases, hospitals are paid considerably less than the billed amount. Hospital charges are based on the type of care provided and can differ from patient to patient for the same service. The differences between patients are related to complications or varying treatments needed due to the patient’s health.

Even though the hospital’s prices on our list are the same for all patients, actual charges for each specific patient may vary from the listed standard charges due to a combination of factors.

    During your Stay

    Daily Groups

Patients participate in daily Mental Health Technician-led groups, focusing on essential areas like life skills, coping mechanisms, mindfulness, and the therapeutic influence of music. They also have the option to join two Recreational Therapy groups, two Clinical Services groups, and two additional nursing groups, each tailored to individual needs.

Patients can access a sensory room, a fully-equipped gym, a library, and a video game room daily. A skilled music therapist visits weekly, enhancing the healing journey with a harmonious touch.

This facility provides a welcoming space where patients can engage in various activities for personal growth and well-being every day.

    Individual Session

Your therapist will regularly meet with you during your treatment to ensure that your goals are being achieved and to address any concerns you may have.

    Family Therapy

Your therapist will contact the family within 24 hours to provide an update, answer any questions and gather any additional needed information. They will schedule a family session within 72 hours of admission. This will provide them a better understanding of the home dynamics which could impact treatment goals. Education on mental health issues can also be provided if needed.

    Medication Management

Your psychiatric team will meet with you to do an initial psychiatric evaluation and then see you throughout your stay to make any necessary adjustments to your medication.
