The first step of the healing journey is letting someone know you need help. Perimeter is here to make this step as easy as possible.
Our process begins with a detailed assessment performed by one of our trained professionals. We will gather some information about you and develop a personalized plan of treatment. When you call our assessment and referral team, we will ask you a few questions in order to schedule a face-to-face appointment. During an in-person assessment, a mental health professional will continue to gather more information with a thorough evaluation including history, current medication, prior treatment, etc. This will help us make a recommendation about the best course of treatment for your unique situation which may be inpatient, outpatient or another treatment option.
You can save time by downloading our “demographic packet” to get a head start on filling out some of the necessary paperwork.
If you are a behavioral health professional looking to set up a direct referral, you can fill out our “direct referral form” to help expedite the admissions process.
Perimeter of Springfield provides face to face assessments in the community for patients and families through their mobile assessment platform. A licensed professional counselor will meet with the child in a safe and secure location to perform a level of care assessment and to make a recommendation for treatment. Often these assessments are for suicidal, homicidal or for aggressive behaviors. If inpatient hospitalization is recommended, transportation to the facility will be arranged. A request for a mobile assessment can be made by calling the assessment and referral department at (417) 799-7474. These assessments are available Monday through Friday from 8:30-5:00 p.m. by request only.
When you reach out to us, we will walk you through the process, answer your questions and help you take that first step. It all begins with a free, confidential assessment by calling 417.799.7474. We are available 24 hours a day, 7 days a week to help determine the best course of treatment for you or someone you love.
