Assessments

Starting the Assessment and Referral Process

The first step of the healing journey is letting someone know you need help. Perimeter is here to make this step as easy as possible.

For our acute inpatient referrals, our process begins with a detailed assessment performed by one of our trained professionals. We will gather some information about you and develop a personalized plan of treatment. When you call our assessment and referral team, we will ask you a few questions in order to schedule a face-to-face appointment. During an in-person assessment, a mental health professional will continue to gather more information with a thorough evaluation including history, current medication, prior treatment, etc. This will help us make a recommendation about the best course of treatment for your unique situation which may be inpatient, outpatient or another treatment option. 

Our process for a residential referral begins by calling 731-668-7073 or our toll-free number 1-800-633-3801. Referrals can also be made via fax on our confidential fax line (870-633-3802). Documents needed to help make a decision on placement include clinical records (current and past treatment records), Individualized Education Plan (IEP -if applicable), outpatient records (if applicable/ex: treatment plan, medication recommendations) and psychological evaluation (completed within the last 2 years). Once the information is received, the clinical team will review and determine the best treatment recommendation for the individual whether it is residential, acute inpatient or outpatient services. If residential treatment is recommended, the facility Referral Coordinator or the Account Manager will coordinate the admission with the appropriate care giver. 

Perimeter Behavioral of Jackson residential referral process often begins with a referral from a behavioral professional including acute inpatient, outpatient providers or other care coordinators. In addition to working with providers in the state of Tennessee, we also have built solid relationships with referral sources throughout the entire United States. Our clinical team will review any referral to ensure we are helping families find the most appropriate level of care for their loved one.
